Not using your pinky!
You should use your pinky as much as possible -- especially in lead guitar!
Not alternate picking!
Use alternate picking! Yes, down picking is cool and heavy, but you should master alternate picking first!
Holding your pick with three fingers!
You should hold your guitar pick only with the thumb and index!
P.S. Don't "anchor" your pick hand fingers anywhere! Anchoring is a bad guitar playing habit beginners develop for stability by bracing your fingers against the body of the guitar near the pick guard area. You are better off without it!
Not using a metronome!

Bad posture!
Practicing with bad physical posture just makes things harder.

Practice more than you think!
You know those little kids that ride around in their parent's cars and say things like: "Are we there yet!?" -- Well, don't say that to yourself!
